Dyspareunia is pain during sexual intercourse. This can have various causes. In women, pain during sexual intercourse can occur continuously and persistently (ie in connection with every sexual intercourse); However, they can also occur for the first time at a later point in time and occur again over time. What are the causes of dyspareunia?
The causes of pain during intercourse can be organic, for example:
- Infections or inflammations (e.g. of the vagina),
- STDs (sexually transmitted diseases)
- Endometriosis,
- Consequences of injuries (e.g. due to childbirth, accidents),
- Consequences of operations (e.g. scarring, adhesions),
- Malformations,
- Tumors,
- dry vagina (e.g. due to hormonal changes during and after menopause) and much more
Various other factors, such as partnership aspects, psychosocial factors, fears or even sexual / sexualised violence, etc., can also be related to pain.
How is the diagnosis made?
The doctor conducts a detailed anamnesis interview in order to get an idea of the patient's physical health. For example, she / he asks about the nature of the pain. Because the pain can be very different, e.g. burning, stabbing, cramping etc.
The doctor also includes possible psychological causes or social burdens (e.g. stress, exhaustion). This also includes questions about the patient's sexual and relationship life. This can be an unfamiliar and possibly unpleasant situation for the woman. Because sexuality is still associated with many taboos that are not always easy to overcome. Ideally, however, both sides can have a conversation that is as open, honest and considerate as possible. A gynecological examination is also carried out.
How is the treatment carried out?
Treatment of dyspareunia depends on the indication. In other words, the treating doctor adjusts the therapy to the individual situation, cause and state of health. If it is based on a somatic (physical) illness, this will be treated as causal if possible. The therapy can therefore be very different. Usually it means the use of medication (such as antibiotics, hormones), less often also surgical interventions etc.
If a physical cause of the complaints can be ruled out, sex medicine specialists, psychotherapists, sex therapists or psychologists can be consulted. A combination of several therapeutic approaches may also be necessary.
Whom can I ask?
The first point of contact is often the general practitioner and the gynecologist. Further medical specialists can be involved in the diagnosis and therapy process.
How are the costs going to be covered?
If there is an illness in the sense of social insurance law, the costs of the medical examination and therapy are covered by the social insurance carriers. Sexual dysfunction is not considered a disease in the sense of social security law. Treatments for sexual dysfunction are not covered by health insurance.
The costs are only taken over by the social security agencies in individual cases.
